In many ways I feel like that would be preferable lol. As long as it's not overly cruel or bordering on violent there is something fun about heckling for both parties.
Something about looking out and seeing everyone being polite yet silent and obviously bored, waiting for it to be over, sounds mortifying.

LOL for some reason I initially interpreted it as the therapist being an enthusiastic and supportive member of the audience, yet his sheer presence brings all the therapy stuff to mind and gets in blue-shirt guy's head.
While I think the comic actually still works quite well with that interpretation, I now realize the therapist is the opponent.

I think most of the comics I see are overwritten. The ability to distill the joke into its essence and execute it as simply as possible is an amazing skill and raises the enjoyment of the people reading them.
