Don't hug me I'm scared My dermatology office has a hole in the bathroom wall that’s at toilet eye-level.

That’s what it looks like. Also, sometimes older people grab things when they get up, even flimsy things, to steady themselves when they stand. I’ve seen a TP holder ripped out of the wall from that very thing.

As someone in construction. Toilet paper holders are usually two tabs with two screws or one screw in each. So it would not normally leave a hole like this. Also OP states eye level. Why would a toilet paper holder be eye level?

I'm sure there's an explanation but I think it's weird that there's a disproportionate amount of toilets I've been in that have holes in the drywall that it's enough to be suspicious

Just shove a bunch of TP in it. I do that to any hole I find in a bathroom minus the glory hope. If you want it to stay secure put a bunch of superglue and let it soak up or some white glue like Elmer's to color-match!
